1.  Comparison of Serological and Nucleic Acid Tests in Hepatitis B and Hepatitis C Blood Donor Screening

Objectives: To compare Hepatitis B virus (HBV) and Hepatitis C virus (HCV) serological test results with the nucleic acid amplification tests in the blood donor patients screening. Methods:During December 2005- January 2008 in Yeditepe University Hospital Microbiology Laboratory, HBV DNA in 136 patients and HCV RNA in 83 patients were tested by real-time PCR and the serums were also tested for HBsAg and anti-HCV respectively by MEIA . Results: Although in HBs Ag positive patients (n=64) HBV DNA was mostly positive, 34% negative patients were also detected(n=22). In 31% of anti-HCV positive patients HCV RNA was positive. In 16% of anti-HCV negative patients (n=61) HCV RNA was positive (n=10). In 72 HbsAg negative patients 6 HBV DNA positivity; in 61 anti-HCV negative patients 10 HCV RNA positivitiy was detected. Conclusion: In routine blood donor HBV and HCV screening, HBV DNA and HCV RNA is recommended to be tested. These tests would be cost effective, not an additional burden.

2. Aneurismal Bone Cyst at Talus

The aneurismal bone cyst, is a benign but local destructive lesion of the bone. It may be formed at any bone; however it is observed in the diaphysis of the longest bones, at vertebra and pelvis. The talus located aneurismal bone cyst is seen very rare. The treated of aneurismal bone cyst is curettage and grefting. Any reoccurrence or complication has not been observed in the 10 months monitoring of our patient. Following the completely curettage of the lesion, it is thought that auto-greft application is a successful method which may be applied in the treatment of talus's aneurismal bone cyst.

3. Modified Bosworth, Revision with Cable and Domino Following the Complication of Screw Loosening

There are money surgical treatment options available for the acromioclavicular joint dislocations. The modified Bosworth technique is an accepted technique due to both facility of application and best results. In the literature the complication of screw loosening reaching to 10% following the modified Bosworth technique has been reported. In our opinion is not reliable to ensure fixation again the same technique as a result of loosening of the screw. The fixation with cable and domino is an easy and reliable technique which may be used also in the primer surgery.

4. Gastrointestinal Involvement of Behcet’s Disease

Behcet was a Turkish dermatologist who first described the triad of relapsing iridocyclitis with recurrent oral and genital ulcerations in 1937. The gastrointestinal manifestations of Behcet's disease (BD) have become recognized as a distinctive type of Behcet's syndrome called intestinal Behcet's syndrome. We describe a rare case of Behçet's disease with rectal ulcer and discuss the difficulties in making a differential diagnosis between Behçet's disease and the inflammatory bowel diseases.

5. Gossypiboma: Unusual Cause of Intestinal Obstruction

Gossypiboma, an infrequent surgical complication, is a mass lesion due to a retained surgical sponge surrounded by foreign-body reaction. Although it is a rare condition, it is a frequent cause of surgical malpractice. Gossypiboma can be asymptomatic clinically or can result in a granulomatous reaction with intraabdominal abscess development, intestinal obstruction, or fistula formation. A 37 year old woman presented with symptoms of small bowel obstruction. She had history of cystocele repair with abdominal approach 4 months ago. Plain abdominal radiography and contrast enhanced abdominal computerized tomography revealed signs of small bowel obstruction and a soft-tissue mass with a dense, enhanced wall, containing internal hyperdense structures in pelvis. Exploratory laparotomy revealed a surgical sponge surrounded by granulation tissue. Postoperative course was uneventful and ileus relieved. On histologic examination, foreign material reaction and focal fibrosis consistent with gossypiboma was detected. Although gossypiboma is rarely seen in daily clinical practice, it should be considered in the differential diagnosis of acute mechanical intestinal obstruction in patients who underwent abdominal surgery previously.
